This gene encodes the extracellular cholesterol esterifying enzyme, lecithin-cholesterol acyltransferase. The esterification of cholesterol is required for cholesterol transport. Mutations in this gene have been found to cause fish-eye disease as well as LCAT deficiency.

Central enzyme for extracellular metabolism of plasma lipoproteins. It is mainly synthesized in the liver and secreted into the plasma to convert cholesterol and phosphatidylcholine (lecithin) into cholesterol esters and lysophosphatidylcholine on the surface of high and low density lipoproteins (HDLs and LDLs). The cholesterol ester is then transported back to the liver. Preference plasma 16:0-18:2 or 18:O-18:2 phosphatidylcholine. It is also produced by primary astrocytes in the brain and esterifies the free cholesterol on the nascent APOE-containing lipoprotein secreted by glial cells, which affects the levels of cerebrospinal fluid (CSF) APOE- and APOA1. Together with APOE and the cholesterol transporter ABCA1, it plays a key role in the maturation of glial-derived nascent lipoproteins. An essential substance to reshape high-density lipoprotein particles into spherical shapes. It catalyzes the hydrolysis of 1-o-alkyl 2-acetyl sn-glycerol 3-phosphocholine (platelet activating factor or PAF) to 1-o-alkyl sn-glycerol 3-phosphocholine (lyso-PAF). It can also catalyze the transfer of acetic acid groups from PAF to 1-hexadecyl alcohol-sn-glycerol-3-phosphocholine to form lyso-PAF. Catalyzes the esterification of (24S)-hydroxycholesterol (24(S)OH-C), also known as cerebrosterol, to produce 24(S)OH-C monoester.

O- and N-glycosylated. O-glycosylation on Thr-431 and Ser-433 consists of sialylated galactose beta 1-->3N-acetylgalactosamine structures. N-glycosylated sites contain sialylated triantennary and/or biantennary complex structures.
