Background
This gene encodes a member of the aquaporin family of intrinsic membrane proteins that function as water-selective channels in the plasma membranes of many cells. The encoded protein is the predominant aquaporin found in brain. Two alternatively spliced transcript variants encoding distinct isoforms have been found for this gene.

Function
Form a specific water channel. It plays an important role in brain water homeostasis and colloidal solute transport. Normal water exchange is required at the blood-brain interface. Cerebrospinal fluid flows along the paravascular space around the perforating artery into the cerebral cortex and brain parenchyma at a normal level, and is necessary for the normal drainage of interstitial fluid along the side drainage channels. Therefore, it is required for the normal removal of solutes in the interstitial fluid of the brain, including APP-derived soluble-amyloid peptides. It plays a redundant role (similarity) in urine homeostasis and urine concentration.
Post-translational modifications
Phosphorylation by PKC at Ser-180 reduces conductance by 50%. Phosphorylation by PKG at Ser-111 in response to glutamats increases conductance by 40% (By similarity).

Anti-aquaporin-4 antibody is involved in the pathogenesis of NMO, a study on antibody titre. Neuromyelitis optica (NMO) is an autoimmune inflammatory disorder of the central nervous system. In most NMO patients, autoantibody to the water channel protein Aquaporin 4 (AQP4) are present at high levels and are thought to drive pathology by mediating complement-dependent destruction of astrocytes.
